The joke is that Trump - who is mad that twitter just censored one of his twitter posts - is acting like the stereotypical "Karen" - an "entitled, obnoxious, middle-aged white woman" (according to dictionary.com)

The reason why this is calling him a Karen is because Karens always want to speak to the manager - it is one of the catch phrases of a Karen.

Since calling someone a Karen is an insult, this is an insult too.
